Greenbrook Elementary PTA Goals for 2017-2018


  1. Offer and build upon events that enrich Greenbrook students' education, health and well being.

  2. Seek opportunities to bring in new volunteers and mentor our future leaders.

  3. To support Diversity in our school and community. Look for new ways to promote diversity through events and activities.

  4. Reach out to new families in our community through an active welcoming committee.

  5. Strive to keep PTA focused on our volunteers and children.

PTA Event Checklist


Warrant Request Form
This form is used for reimbursement after events and to release funds before an event. 


Deposit Verification Form
This form is to be used to deposit your funds from an event. Please put in PTA Financial Secretary file in the front office. If your deposit contains cash, make arrangements to deliver it personally.


Copy Request Form
Please use this form to request copies be made for flyers and documents for PTA sponsored events and meetings.


Honorary Service Awards Form


Family Waiver and Family Waiver (Spanish)
This form is the "Parent's Approval and Student/Participant's Waiver" and is for parents to complete when dropping their kids off at PTA sponsored events (i.e. Movie Night, etc.).
